Shoot wavers?

Just checking what other clubs do? Just finished our annual shoot am down -$175 for all the sporting clays -skeet - and trap, plus food and drink from 10:00 to 4:00. Ran the trap range and seen to many uneducated shooter -Safety violation, and that was on trap alone. Sporting clays course is much harder to police. With insurance being a big issue, are clubs requiring any type of waiver or form to sign? Any ideas or sample would be great?
